Medical (ENG1) valid for 1 year from issue date
GWO Working at Height
GWO Sea survival
HSE Medic certificate
Colour copy of Seamans Book (first two photo spread pages) (also to provide last full years sea time if they have it)
Colour copy of passport (double spread page of passport)
